GENERAL INSTRUCTIONS

To receive Settlement Benefits, a claim form should be filled out online or submitted by mail if you received a notification from Methodist McKinney Hospital ( “MMH”) relating to a cyberattack against MMH’s computer systems that occurred between May 20, 2022 and July 7, 2022 (the “Data Security Incident”), and you had documented out-of-pocket losses, or lost time spent dealing with the Data Security Incident, or you wish to claim a pro rata cash payment and/or credit monitoring and identity protection services to be paid for by Defendants. You may get a payment if you fill out this claim form, if the settlement is approved, and if you are found to be eligible for a payment.

To start your online claim form, Claim Login ID and PIN at the top of your postcard Notice. If you are unable to locate your ID & PIN, please contact us by emailing info@MMHSettlement.com. Please request your Claim Login ID & PIN by providing the Class Member’s full name and mailing address.
